The US Dept. of Education has a booklet for high school teacher for transition students with disabilities who are interested in postsecondary education.  This question and answer booklet can be ordered through the Dept. of Ed website and will reach you in less than 10 days.  I recommend all high school go to the site and order this free publication.

If you have not visited the National Technical Assistance Center on Transition (NTACT) in a while, I suggest you do it.  It offers evidence-based lesson plan starters at the secondary level.
